End-of-life care services
Our Patient Care Teams consist of physicians, registered nurses, certified nursing assistants, social workers, chaplains, music therapists, and volunteers—all dedicated to helping our patients live every day that remains as fully as possible.
- Receive care at home: We provide care wherever a patient calls home.
- Comprehensive care: We ensure patients have the equipment, supplies, and medicines they need.
- Inpatient Care Center: If a patient has symptoms or pain that cannot be managed effectively at home, an elevated level of care is available at our Inpatient Care Center where patients receive short-term, round-the-clock care in a homelike setting with their loved ones by their side.
- Caregiver support & respite care: We also support our patient’s caregivers and family by providing education, bereavement services, and up to 5 days a month of Respite Care at our Inpatient Care Center.
Additional services
Our social workers and chaplains provide emotional and spiritual support to patients and families every step of the way.
Our unique Music Therapy program, led by board-certified music therapists, is the only one of its kind in our area. Music therapy addresses physical, social, emotional, and spiritual needs with the goal of increasing quality of life, decreasing pain, alleviating spiritual suffering, and promoting relaxation.
We are a proud We Honor Veterans partner, offering specialized veteran-focused end-of-life care services.
Our team of dedicated trained volunteers can provide a variety of support services including pet therapy, Reiki therapy, tuck-in calls, 1-on-1 support and companionship, help with things like groceries and transportation, personal care, and sitting vigil in the final moments of life.
