RN On Call

Part Time Position

  • Position involves travel throughout our territory to patient homes providing direct patient care and education to hospice patients and their caregivers
  • Current RN state license required with a minimum of 2 years acute care experience
  • Previous hospice experienced preferred
  • Must have excellent assessment and problem-solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
  • Position provides telephone triage symptom management and focused in home visits
  • Requires ability to drive throughout Blue Ridge Hospice service area
  • Willingness to work either evening or night shift and rotating weekends

Hospice Physician

Part Time Position

  • Responsible for taking all admission calls while on duty in a timely manner
  • Being familiar with hospice local coverage determination criteria and CMS guidelines for hospice eligibility
  • Reviewing medical records of potential admissions in advance and collaborating with assessment RN’s to determine appropriate primary hospice diagnosis
  • Determining covered and non-covered diagnoses and medications, substituting for formulary medications when appropriate
  • Addressing acute medical symptoms present during assessment visit (should pt be appropriate for admission) by collaborating and problem solving with RN via telephone
  • Signing orders for comfort kits as needed
  • Signing orders for medications generated from admission process
  • Ordering any necessary DME for new admissions
  • Review via telephone with assessment RN any current hospital patients to determine hospice eligibility
  • Collaborate with hospice providers or community referral sources if needed
  • Becoming familiar with proficient in current EMR
  • No direct patient time
  • Work in office setting collaborating with trained hospice RN’s in cutting edge access program

  • Current RN state license required
  • Position requires minimum of 2 years current acute care experience
  • Must have excellent assessment and problem-solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
  • Previous hospice experience preferred

LPN – ICC – PRN

  • Serves patients in our Inpatient Care Center
  • 12-hour shift, days or nights
  • Must have excellent assessment and problem-solving skills
  • Current unrestricted Virginia LPN license
  • Must be familiar with general use and functions of the computer
  • Must have the ability to document care provided in an Electronic Medical Record accurately and in a timely manner
  • Position requires minimum of 2 years current acute care experience
  • Previous hospice experienced preferred

Nurse Educator

Full Time Position (40 hours)

  • Oversees the development, planning, presentation and evaluation of educational activities within areas of expertise which foster’s the development of confidence in the learner
  • Provides and supports professional practice activities to achieve organizational goals
  • Oversees orientation of new employees including development and maintenance of weekly orientation objectives and monitoring each employee’s progress
  • Supports agency wide professional practice education programs.
  • Serves to enhance staff retention by role modeling professional practice
  • Graduate of an accredited nursing school; licensed as a Registered Nurse in the Commonwealth of Virginia
  • Master’s degree in nursing preferred
  • Previous experience in clinical education preferred
  • Two or more years of relevant nursing experience in terminal care, health team service coordination, documentation and case management. Possess previous experience in hospice or home health preferred
  • A strong knowledge of hospice regulations and reimbursement issues surrounding hospice care preferred
  • Ability to manage all aspects of terminal care, health team service coordination, and documentation preferred
  • Knowledgeable of organizational policies, procedures, systems and objectives
  • Have strong communication skills; written and verbal
  • Be able to identify quality assurance triggers and take the lead on establishing education plans

LPN Home Care

Full Time Position (40 hours)

  • Position requires minimum of 2 years current acute care experience
  • Previous hospice experienced preferred

RN Case Manager

Full Time Position (40 hours)

Chief Development Officer

Full Time Position (40 hours)

  • Responsible for the planning and implementation of all development programs
  • Serves as the Director for the Blue Ridge Hospice Development Council
  • Identifies, cultivates, and solicits prospective and current donors; builds relationships and generates major/annual gift and pledges in order to reach Development Council goals
  • Oversees and manages the Development Council and employee giving programs, thereby organizing committee meetings, soliciting employee and small gift donors
  • Develops, implements, monitors and evaluates programs and activities to meet the goals of annual and strategic plans and priorities.
  • Assumes key role in fundraising activities, which includes but is not limited to, annual fund, capital campaign, and donor recognition
  • Oversees activities of the Butterfly Club
  • Works to get the CEO invited to all community events/meetings
  • Works to make the CEO visible and remain the face of the organization
  • Meets or exceeds annual team fundraising goal.
  • Oversees potential grant resources to create appropriate grant proposals
  • Conducts research on gift potential with individuals, corporations, and foundations, then uses this information to develop rapport with potential special gift donors
  • Informs potential contributors of any special projects of the organization. Encourages individuals/and or corporations to contribute to special projects
  • Identifies key media audiences and provides routine and timely issue related media releases, PSA’s and development opportunities
  • Ensures that promotional tools and printed materials are produced, updated periodically and disseminated to the appropriate audience
  • Ensures production of the newsletter, literature, publications and materials for external distribution
  • Represents Blue Ridge Hospice at area Chamber of Commerce meetings, networking meetings and other community events
  • Manages the website ensuring that it is updated and reflects a consistent Hospice image. Acts as staff photographer and editor, preparing all photos for online and print use
  • Manages updates on various social media sites to stay current, engaging “followers”, and marketing the organization
  • Coordinates responses to requests from the professional community for all staff speakers ensuring a professional and consistent message as well as speaks publicly to educate the community regarding the organization, its mission and overall needs
  • Oversees all special events for Hospice to include galas, auctions, etc.
  • Oversees the Donor Services department and monitors confidentiality of privileged management and donor information
  • Participates in the planning and budgeting for long term viability.
  • Respond, promptly, courteously and respectfully to donors through letters, phone calls and visits
  • Attends quarterly Board of Directors meetings, as needed.
  • Provides feedback, instruction, and development guidance to help team members excel in their current or future job responsibilities
