Hospice Physician

Part Time Position

  • Provides hospice homecare patient visits
  • Participates in interdisciplinary hospice home care team meetings
  • Evaluation for hospice certification of terminal prognosis of patients, quality of initial plans of care and comprehensive plans of care, revocations, re-certification of terminal prognosis, quality of pain and symptom management
  • Ensures accurate documentation of patient care to support reimbursement for services provided
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization
  • Participate in on-call rotation as needed
  • The physician has the responsibility for the medical care and treatment of patients rendered by the hospice interdisciplinary team (IDT) and shall consult and cooperate with the patient’s attending physicians
  • The physician provides direction and guidance to the staff and volunteers of the hospice IDT to assure the maintenance of quality standards of care for patients and families
  • Actively participates in the clinical education and training for hospice patient care, admissions, and administrative personnel
  • Doctor of Medicine or Osteopathy licensed to practice medicine in the state of Virginia
  • Board certification in a primary care specialty, Board certified, or eligible, in Hospice and Palliative care
  • Unrestricted narcotic DEA license
  • Proven track record with interdisciplinary teams, effectively establishing goals of care and building collaborative physician/patient/family relationships in addition to knowledge of hospice and palliative care, pain management and medical ethics preferred
  • Intermediate computer knowledge
  • Previous hospice care experience preferred

RN Assessment

Full Time Position (40 hours)

  • Performs the assessment function and derives nursing diagnoses based on assessment data.  Coordinates assessment visits with Family Support Services
  • Documents assessment & clinical information in electronic medical record
  • Provides Plan of Care to the team members utilizing voice care system.
  • May act as liaison with WMC and WMH, monitoring in-patients to ensure appropriate care and treatments are authorized by BRH until symptoms are controlled
  • Shares responsibility for weekend rotation.
  • RN License required.  Graduate of an NLN accredited nursing program, Bachelor of Science in Nursing preferred
  • Hospice experience strongly preferred
  • Position requires minimum of 2 years current acute care experience

RN Float

  • Position involves travel throughout our territory to patient homes providing direct patient care and education to hospice patients and their caregivers
  • Current RN state license required
  • Position requires minimum of 2 years current acute care experience

Assistant Chief Nursing Officer

Full Time Position

  • Evaluates, and documents the effectiveness of care in conjunction with other members of the interdisciplinary team (IDT), to include patient, family, physician and facility staff.
  • Provides for continuity of service across settings through effective information management, including both verbal and written communication.
  • Delivers care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.  Articulates essential components of the hospice benefit as administered under Medicare and other payers.
  • Conducts all performance evaluations for Clinical Team Leaders and Clinical Staff with input from the Clinical Team.
  • Participates in infection control activities, education and monitoring as needed.
  • Provides clinical/administrative support and leadership to the clinical staff in the absence of Clinical Team Leaders.
  • Ensures all documentation and nursing services meet regulatory, federal hospice guidelines, and organization policies and procedures.
  • Serves as a resource/mentor for staff regarding clinical issues, documentation, and problem solving.
  • Communicates with clinical staff regularly/effectively regarding organizational changes and updates, practice expectations and other matters of importance.
  • Provides guidance and support to afterhours/on-call services, as necessary.
  • Provides all aspects of direct patient care during times of limited resources/high acuity including:  completing admission assessments, completing patient assessments in the home or facility setting, triaging patient calls, covering for assigned staff during absences, etc.
  • Licensed Registered Nurse with current Virginia licensing per state board of nursing
  • Certification in Hospice and Palliative Care nursing preferred
  • Two years hospice experience preferred
  • Bachelor’s degree in nursing preferred
  • Strong leadership and management experience in effective team building
  • Reliable transportation

RN On Call

Full Time & Part Time (24 hours)

  • Position involves travel throughout our territory to patient homes providing direct patient care and education to hospice patients and their caregivers
  • Current RN state license required with a minimum of 2 years acute care experience
  • Previous hospice experienced preferred
  • Must have excellent assessment and problem-solving skills
  • Ability to work within an interdisciplinary team setting
  • Must be familiar with general use and functions of the computer
  • Position provides telephone triage symptom management and focused in home visits
  • Requires ability to drive throughout Blue Ridge Hospice service area
  • Willingness to work either evening or night shift and rotating weekends

Team Leader, Clinical Services

Full Time Position

  • Attend Team Leader and Administrative quarterly meetings and bi-weekly manager meetings.
  • Assures documentation of care provided and the response of the patient/family; assures submission of documentation as described in organization policy.
  • Provides for continuity of service across settings through effective information management, including both verbal and written communication.
  • Delivers care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies.  Articulates essential components of the hospice benefit as administered under Medicare and other payers.
  • Manages patient related telephone calls.
  • Participates in infection control activities, education and monitoring as needed.
  • Provides clinical/administrative support and leadership to the clinical staff in the absence of other Team Leaders and Assistant Chief Nursing Officer if assigned.
  • Monitors Quality Department reports are followed up on with employees.
  • Reviews/monitors/approves log sheets to include management of staff overtime.  Reviews patient care expenditures to ensure covered versus non-covered expenses.  Monitors productivity standards.
  • Serves as a resource/mentor for staff regarding clinical issues, documentation, and problem solving.  Refer staff to help desk when needed.
  • Communicates with clinical staff regularly/effectively regarding organizational changes and updates, practice expectations and other matters of importance.
  • Carries a patient caseload 50% of the BRH established caseload per discipline.  This caseload will increase to 75% once the individual is trained in this role.  Will spend 75% of time in the field and 25% administration.
  • Provides guidance and support to after-hours/on-call services, as necessary.  This is handled on a rotating basis with other Team Leaders/Managers.
  • Coordinates all CNA services.  Monitors schedules for aide services to meet patient/family needs while considering required supervisory visits. Notifies aides, nurses and patients of changes to schedule, if necessary.
  • Provides all aspects of direct patient care during times of limited resources/high acuity including:  completing admission assessments, completing patient assessments in the home or facility setting, triaging patient calls, covering for assigned staff during absences, etc.
  • Approves payroll bi-weekly.  Approves paid time off and monitors team paid time off.
  • Facilitates IDT team meetings.  Does not act as Scribe.  Assist keeping meeting moving, timely and appropriate.
  • Participates in the orientation of new members of the IDT and students.
  • Monitors staff compliance with all regulatory requirements and Blue Ridge Hospice policies and procedures and ensures appropriate staff education.  Identifies any education needs or opportunities as needed.
  • Utilizes Consolo software program and becomes a Super User for the program.  Acts as a resource to the team for clinical questions.
  • Possesses interpersonal skills that promote effective teamwork and communication.
  • Licensed Registered Nurse or Social Worker with current Virginia license.
  • Two years hospice experience preferred.
  • Bachelor’s degree preferred.
  • Strong leadership and management experience in effective team building.

Thrift Associate, Leesburg

Full Time Position (40 hours)

  • Must demonstrate merchandising skills/experience
  • Must possess excellent customer service skills
  • Must have prior experience with cash handling and opening & closing stores
  • Position will include sorting & pricing
  • Position requires evening hours and weekends
  • Must be able to lift up to 50 pounds and stand for long periods of time

RN Case Manager

Full Time Position (40 Hours)

Nurse Educator

Full Time Position

  • Oversees the development, planning, presentation and evaluation of educational activities within areas of expertise which foster’s the development of confidence in the learner
  • Provides and supports professional practice activities to achieve organizational goals
  • Oversees orientation of new employees including development and maintenance of weekly orientation objectives and monitoring each employee’s progress
  • Supports agency wide professional practice education programs.
  • Serves to enhance staff retention by role modeling professional practice
  • Graduate of an accredited nursing school; licensed as a Registered Nurse in the Commonwealth of Virginia
  • Master’s degree in nursing required
  • Previous experience in clinical education preferred
  • Two or more years of relevant nursing experience in terminal care, health team service coordination, documentation and case management. Must possess previous experience in hospice or home health
  • Must have a strong knowledge of hospice regulations and reimbursement issues surrounding hospice care
